We are so grateful for the opportunity to partner with Agrace for the implementation of their Salesforce CRM system, supporting their mission to improve the quality of life for patients and families throughout serious illness. Agrace is a nonprofit, community-based health care agency that provides hospice and supportive care services in private homes and in skilled nursing, assisted living and other community residential facilities across southern Wisconsin.
In order to better manage relationships with health care providers, health care facilities and others who partner to provide access to Agrace’s programs and services, it became essential to have a centralized database that provided a single view of each organization.
